Johns Hopkins researchers have uncovered strong evidence that mice have a specific set of nerve cells that signal itch but not pain, a finding that may settle a decades-long debate about these sensations, and, if confirmed in humans, help in developing treatments for chronic itch, including itch caused by life-saving medications.

At the heart of their discovery is a type of sensory nerve cell whose endings receive information from the skin and relay it to other nerves in the spinal cord, which then coordinates a response to the stimulus. Published online Dec. 23 in Nature Neuroscience, a report on the research suggests that even when the itch-specific nerve cells receive stimuli that are normally pain-inducing, the message they send isn't "That hurts!" but rather "That itches!"

Pain and itch are both important sensations that help organisms survive. And pain is arguably more important because it tells us to withdraw the pained body part in order to prevent tissue damage. But itch also warns us of the presence of irritants, as in an allergic reaction. However, "when either of these sensations continues for weeks or months, they are no longer helpful. We even see patients stop taking life-saving medications because they cause such horrible itchiness all over," says Xinzhong Dong, a Howard Hughes early career scientist and associate professor of neuroscience at the Institute of Basic Biomedical Sciences at the Johns Hopkins University School of Medicine. "And sometimes when we try to suppress chronic pain, with morphine for example, we end up causing chronic itchiness. So the two sensations are somehow related, and this study has begun to untangle them," he says.

Because nerve cells send their messages as electrical currents that flow through them just as they would through wires, scientists can plug tiny monitors into individual nerve cells to detect the moment of stimulation. The scientific controversy over pain and itch centers around a group of nerve cells known to respond electrically to painful stimuli such as molecules of capsaicin, the fiery ingredient in chili peppers. A small subset of these nerve cells also responds electrically to itchy stimuli because they have on their surfaces receptors for molecules like histamine. One of these itchy receptors, called MrgA3, binds the anti-malaria drug chloroquine, causing serious itchiness in many patients.

Sensory nerve scientists have not known whether the nerves with itchy receptors and pain receptors were actually sending both types of messages to the brain, or just itch messages. What the current study found is that, in nerves with the itchy receptor MrgA3, electrical signals sent in response to both painful and itchy stimuli are interpreted by the brain as itch.

To reach this conclusion, the researchers first used a genetic trick to label the MrgA3 cells in mice with a glowing protein that allowed them to see the cells under the microscope. Aided by the glow, they were able to plug in those tiny electricity monitors and watch nerve cell responses to different stimuli. The cells transmitted electrical signals when the mice were exposed to itch-inducing chloroquine and histamine, as well as pain-inducing capsaicin and heat. Based on this result, the researchers tentatively concluded that the cells could send both pain and itch signals.

In the next experiment, the researchers monitored the behavioral responses of mice to the different stimuli. As expected, when the tails of normal mice were placed in hot water, they quickly pulled them out; when normal mice were given a bit of chloroquine or histamine, they scratched vigorously with their hind legs.

Then, to examine the role of MrgA3 cells in pain and itch, the scientists selectively killed MrgA3 nerve cells in adult mice and retested their responses. Presumably, the researchers noted, because MrgA3 cells are only a small fraction of all pain-sensing nerve cells, the mice had normal withdrawal responses to painful stimuli like hot water. However, when exposed to itchy stimuli, their scratching responses were reduced to varying degrees depending on the stimulus, most significantly in response to chloroquine. The fact that some stimuli still caused scratching suggested to the scientists that MrgA3 cells are not the only ones in the body that respond to itch. "We were convinced that MrgA3 cells are responsible for much of the sensation of itch, but it wasn't yet clear whether MrgA3 cells could also relay painful information," says Dong.

In their final experiments, the scientists used genetic techniques to create mice in which the MrgA3 cells were the only cells in the body capable of responding to capsaicin, that peppery pain-inducing substance. When injected into the cheeks of mice, normal mice massage the area with their forepaws to relieve the hot sensation. When injected into the experimental mice, they vigorously scratched their cheeks with their hind legs, suggesting that this normally painful stimulus had been communicated to the brain ? by MrgA3 cells ? as itchiness.

"Now that we have disentangled these itchy sensations from painful ones, we should be able to design drugs that target itch-specific nerve cells to combat chronic itchiness," says Dong. "We hope that this will not only provide relief, but also increase people's faithfulness to their drug plans, particularly for deadly diseases like malaria and cancer."

Will Indian gang-rape death lead to reforms?

In response to the death of a young woman after a violent sexual attack in India, protestors are taking to the streets. They have also submitted a petition asking for politicians facing charges of similar crimes to be removed from office.

Indian lawmakers facing sexual assault charges could be suspended from office if the country's top court rules in favor of a petition submitted after a gang-rape and murder that shocked the country.

Six state lawmakers are facing rape prosecutions and two national parliamentarians are facing charges of crimes against women that fall short of rape, said Jagdeep S. Chhokar, an official with the Association for Democratic Reforms, which tracks political candidate's criminal records.

The petition will be heard Thursday, the same day police plan to formally charge six suspects in the attack on a 23-year-old university student in New Delhi two weeks ago.

The rape triggered outrage and sparked demands for stronger laws, tougher police action against sexual assault suspects and a sustained campaign to change society's views on women.

As part of that campaign, Chief Justice Altamas Kabir agreed to hear a petition from retired government administrator Promilla Shanker asking the Supreme Court to suspend all national and state lawmakers who are facing prosecution for crimes against women.

She also asked the court to force the national government to fast-track thousands of rape cases languishing in India's?notoriously sluggish court system.

In the past five years, political parties across?India?nominated 260 candidates awaiting trial on charges of crimes against women, Chhokar said. Parties ran six candidates for the national parliamentary elections facing such charges, he said.

"We need to decriminalize politics and surely a serious effort has to be made to stop people who have serious charges of sexual assault against them from contesting elections," said Zoya Hasan, a political analyst.

On Wednesday morning, several thousand women held a silent march to Gandhi's memorial in the capital in memory of the victim, holding placards demanding "Respect" and "Justice." Delhi Chief Minister Sheila Dikshit joined the women for a prayer session for the victim. The Gandhi memorial is a common protest site.

On Tuesday, the government set up a task force to monitor women's safety in New Delhi and to review whether police were properly protecting women. Two task forces already are examining the handling of the rape case and possible changes in rape laws.

The rape of the unidentified woman on a bus in the capital has horrified many and brought unprecedented attention to the daily suffering of women here, who face everything from catcalls and groping to rapes.

Six men arrested in the case were to be formally charged Thursday with kidnapping, rape and murder, said Rajan Bhagat, the New Delhi police spokesman. Police have said they would push for the death penalty. Another suspect underwent medical testing to determine his age since juveniles cannot be charged with murder in?India.

The Bar Association of lawyers last week decided against defending the six suspects because of the nature of the crime, although the court is expected to appoint attorneys to defend them.

Media reports say 30 witnesses have been gathered, and the charges have been detailed in a document running more than 1,000 pages. Police also have detained the owner of the bus used in the crime on accusation he used false documents to obtain permits to run the private bus service.

The family of the victim ? who died Saturday at a hospital in Singapore ? is struggling to come to grips with the tragedy.

"She was a very, very, very cheerful little girl and she was peace loving and she was never embroiled in any controversies like this. I don't know why this happened to her," her uncle, Suresh Singh, told The Associated Press on Wednesday.

The family of the victim, whose name was not revealed, called for stronger rape laws to prevent such attacks from happening again and demanded swift ? and harsh ? justice for woman's assailants, Singh said.

"If the government can't punish them, give the rapists to the people. The people will settle the scores with them," he said.

Amazon Won A Legal Battle Against Apple - Business Insider

Jan. 2 (Bloomberg) -- Amazon.com Inc. won dismissal of Apple Inc.?s claim that the online retailer?s use of the term ?app store? for Android device software is false advertising.

U.S. District Judge Phyllis Hamilton in Oakland, California, today granted Amazon?s request to throw out one claim in Apple?s lawsuit alleging trademark infringement and unfair competition over the Amazon Appstore for Android, a service begun in March that sells applications for the Kindle Fire and devices running Google Inc.?s Android software.

Apple is seeking a court order to block Amazon from using the term. Amazon has argued the words are a generic term that Apple doesn?t have exclusive rights to use. A trial is scheduled for Aug. 19.

The iPhone maker, which started its APP STORE in 2008, said Amazon?s use of the term was false advertising because it deceives customers into believing that Amazon?s service has the qualities of Apple?s applications store, Hamilton said in her ruling. Apple maintained this could divert its revenue to Amazon.

Apple hasn?t shown that Amazon?s advertising attempt to mimic Apple?s, failed to show that Amazon made any false statement, and presented no evidence that customers were misled by Amazon?s use of the term, Hamilton said.

?The court finds no support for the proposition that Amazon has expressly or impliedly communicated that its Appstore for Android possesses the characteristics and qualities that the public has come to expect from the Apple APP Store and/or Apple products,? Hamilton said.

She ruled only on Amazon?s request to eliminate the false advertising claim.

In 2008, Apple applied to the U.S. Patent and Trademark office to register APP STORE, according to the order. Microsoft Corp. opposed the registration, saying the term is generic. Last year the Trademark Trials and Appeals Board put an opposition proceeding on hold pending the outcome of the lawsuit before Hamilton.

Kristin Huguet, a spokeswoman for Cupertino, California- based Apple, declined to comment on the ruling. A call to Seattle-based Amazon?s media line seeking comment on the ruling wasn?t immediately returned.

The case is Apple Inc. v. Amazon.com Inc., 11-01327, U.S. District Court, Northern District of California (Oakland).

Gerard Depardieu's latest drama: a Russian passport

Russian President Vladimir Putin has given a Russian passport to the famous French movie star in what some see as part of an escalating war of words between Russia and the West.

A terse announcement posted on the Kremlin website?noted that Mr. Putin acted "to satisfy an application for citizenship of the Russian Federation by Gerard Xavier Depardieu, who was born in 1948 in France."

Mr. Depardieu, star of over 170 films and possessing what is often politely referred to as a "colorful" public personality, has been locked in a high profile battle with France's new socialist government over an emergency tax that would levy a 75 percent rate on people earning more than $1.3 million. He recently renounced his French citizenship and took up residence in Belgium, which offers a friendlier tax regime for the super-rich.

France's high court struck down the law last week as "unconstitutional," but the government announced it will soon reintroduce the measure after taking the court's concerns into account.

It's not clear whether Depardieu actually applied for residence in Russia, which has a 13 percent flat income tax for all, but in a far-ranging press conference?last month Putin declared "If G?rard really wants to have a residence permit or a Russian passport, you can consider it done, the issue solved positively."

Putin also said that he has long enjoyed "kind, friendly, personal relations" with the French actor.

Come to Chechnya

Depardieu is no stranger to Russia. He has appeared in several ad campaigns and filmed the 2011 movie Rasputin in St. Petersburg. He is also rumored to be close to Chechen strongman Ramzan Kadyrov, and was guest of honor at the pro-Kremlin leader's birthday party in Grozny last October.

Mr. Kadyrov has said that he would happily invite Depardieu to come and live permanently in Chechnya if he wanted to. "If the country's leadership decides in favor of granting Depardieu Russian citizenship, we will be glad to create deserved conditions for the great cultural figure in our republic," Kadyrov said last week.

Answers for Special Kids - 10 Basic Steps in Special Education

Children can have all sorts of difficulties growing up. Sometimes problems are obvious right from the start; and sometimes they don?t appear until a child is in school. Some children have trouble learning to read or write. Others have a hard time remembering new information. Still others may have trouble with their behavior. For some children, growing up can be very hard to do!

When a child is having trouble in school, it?s important to find out why. The child may have a disability. By law, schools must provide special help to eligible children with disabilities. This help is called special education and related services.

There?s a lot to know about the process by which children are identified as having a disability and in need of special education and related services. This section of NICHCY?s website is devoted to helping you learn about that process.

This brief overview is an excellent place to start. Here, we?ve distilled the process into 10 basic steps. Once you have the big picture of the process, it?s easier to understand the many details under each step. We?ve indicated throughout this overview where, on our site, you can connect with that more detailed information.

Step 1. Child is identified as possibly needing special education and related services.

There are two primary ways in which children are identified as possibly needing special education and related services: the system known as Child Find (which operates in each state), and by referral of a parent or school personnel.

Child Find. Each state is required by IDEA to identify, locate, and evaluate all children with disabilities in the state who need special education and related services. To do so, states conduct what are known as Child Find activities.
When a child is identified by Child Find as possibly having a disability and as needing special education, parents may be asked for permission to evaluate their child. Parents can also call the Child Find office and ask that their child be evaluated.

Referral or request for evaluation. A school professional may ask that a child be evaluated to see if he or she has a disability. Parents may also contact the child?s teacher or other school professional to ask that their child be evaluated. This request may be verbal, but it?s best to put it in writing.

Parental consent is needed before a child may be evaluated. Under the federal IDEA regulations, evaluation needs to be completed within 60 days after the parent gives consent. However, if a State?s IDEA regulations give a different timeline for completion of the evaluation, the State?s timeline is applied.

Step 2. Child is evaluated.

Evaluation is an essential early step in the special education process for a child. It?s intended to answer these questions:

  • Does the child have a disability that requires the provision of special education and related services?
  • What are the child?s specific educational needs?
  • What special education services and related services, then, are appropriate for addressing those needs?

By law, the initial evaluation of the child must be ?full and individual??which is to say, focused on that child and that child alone. The evaluation must assess the child in all areas related to the child?s suspected disability.

The evaluation results will be used to decide the child?s eligibility for special education and related services and to make decisions about an appropriate educational program for the child.

If the parents disagree with the evaluation, they have the right to take their child for an Independent Educational Evaluation?(IEE). They can ask that the school system pay for this IEE.

Step 3. Eligibility is decided.

A group of qualified professionals and the parents look at the child?s evaluation results. Together, they decide if the child is a ?child with a disability,? as defined by IDEA. If the parents do not agree with the eligibility decision, they may ask for a hearing to challenge the decision.

Step 4. Child is found eligible for services.

If the child is found to be a child with a disability, as defined by IDEA, he or she eligible for special education and related services. Within 30 calendar days after a child is determined eligible, a team of school professionals and the parents must meet to write an individualized education program (IEP) for the child.

Step 5. IEP meeting is scheduled.

The school system schedules and conducts the IEP meeting. School staff must:

  • contact the participants, including the parents;
  • notify parents early enough to make sure they have an opportunity to attend;
  • schedule the meeting at a time and place agreeable to parents and the school;
  • tell the parents the purpose, time, and location of the meeting;
  • tell the parents who will be attending; and
  • tell the parents that they may invite people to the meeting who have knowledge or special expertise about the child.

Step 6. IEP meeting is held and the IEP is written.

The IEP team gathers to talk about the child?s needs and write the student?s IEP. Parents and the student (when appropriate) are full participating members of the team. If the child?s placement (meaning, where the child will receive his or her special education and related services) is decided by a different group, the parents must be part of that group as well.

Before the school system may provide special education and related services to the child for the first time, the parents must give consent. The child begins to receive services as soon as possible after the IEP is written and this consent is given.

If the parents do not agree with the IEP and placement, they may discuss their concerns with other members of the IEP team and try to work out an agreement. If they still disagree, parents can ask for mediation, or the school may offer mediation. Parents may file a state complaint with the state education agency or a due process complaint, which is the first step in requesting a due process hearing, at which time mediation must be available.

Step 7. After the IEP is written, services are provided.

The school makes sure that the child?s IEP is carried out as it was written. Parents are given a copy of the IEP. Each of the child?s teachers and service providers has access to the IEP and knows his or her specific responsibilities for carrying out the IEP. This includes the accommodations, modifications, and supports that must be provided to the child, in keeping with the IEP.

Step 8. Progress is measured and reported to parents.

The child?s progress toward the annual goals is measured, as stated in the IEP. His or her parents are regularly informed of their child?s progress and whether that progress is enough for the child to achieve the goals by the end of the year. These progress reports must be given to parents at least as often as parents are informed of their nondisabled children?s progress.

Step 9. IEP is reviewed.

The child?s IEP is reviewed by the IEP team at least once a year, or more often if the parents or school ask for a review. If necessary, the IEP is revised. Parents, as team members, must be invited to participate in these meetings. Parents can make suggestions for changes, can agree or disagree with the IEP, and agree or disagree with the placement.

Step 10. Child is reevaluated.

At least every three years the child must be reevaluated. This evaluation is sometimes called a ?triennial.? Its purpose is to find out if the child continues to be a child with a disability, as defined by IDEA, and what the child?s educational needs are. However, the child must be reevaluated more often if conditions warrant or if the child?s parent or teacher asks for a new evaluation.
